Foundation Funding Request
The mission of the Colorado Health Initiatives Colorado Foundation is to inspire financial and spiritual support of CommonSpirit Health’s mission by building personal relationships and community engagement. To steward our benefactors’ gifts wisely, the following criteria will be weighed to ensure the greatest impact of the entrusted philanthropic dollars and proper use of available funds.
In order to prepare individuals to successfully submit the funding form, we invite you to read on.
Does the program/project support the CommonSpirit Health Mission?
As CommonSpirit Health, we make the healing presence of God known in our world by improving the health of the people we serve, especially those who are vulnerable, while we advance social justice for all.
Does this program/project meet one or more of the identified funding priorities of the CommonSpirit Health Mountain region?
Foundation Funding Priorities include: Behavioral Health, Cancer Care, Community Health, Education/Training, Hospice and Home Health, Heart and Vascular Care, Orthopedics, Neurology, Women’s Health
Is this program/project a priority project for a hospital, ministry, or service line?
Approval will be required by hosptial leadership and additional approvals may be may include Finance or other departments.
How many patients/caregivers will be directly impacted?
Does this program/project serve the uninsured/underserved community?
Does this project/program impact revenue? If so, has a pro forma been submitted?
Be prepared to share the impact. We regularly share with our benefactors and report on our collective effort. Be prepared to supply the following components: stories/photos illustrating the direct impact on the lives of patients, including long-term benefits, including qualitative and quantitative information
Additional items to consider
- Funding Requests are reviewed by the Foundation on a quarterly cycle
- Requests requiring urgent funding will be considered on an as-needed basis.
- Project Budget or Quote - Quotes must be provided by a CommonSpirit Health approved vendor
